Necessary Documents
- Personal Identification. Either a social security card and a driver’s license
- Medicare Card
- Supplemental Insurance (if needed)
- Power of Attorney or Durable Power of Attorney documents
- Guardianship Documents (if applicable)
- Living Will and/or DNR Documents
- Medical Records from Primary Physician
- Physician Admissions Orders
The nursing home admission process can be overwhelming, especially when juggling the emotional impact of placing a loved one in a nursing home. Knowing what to expect can help alleviate some of the stress. Our Admissions Coordinator is here to help make the admission process as easy as possible. Once a room is selected a security deposit is made to hold the room and documentation from the applicant’s primary care physician will be obtained.
Admission to the facility can take place once all these documents are in place. If you have any questions, please don’t hesitate to contact social services.
All documents for admission will be done on the day the patient is admitted to the facility. The financial agreement and the first 30 days of rent will be due upon admission to the facility.
